The Approach
A filler adds volume. A biostimulator makes you grow your own. Those are not the same purchase, and one of them cannot be undone.
This is the distinction almost nobody explains, and it is the one that matters most. A hyaluronic acid filler is volume in a syringe. It goes in, you see the result that day, and if you hate it, it can be dissolved with an enzyme. It is immediate and it is reversible. Sculptra and Radiesse are biostimulators. They do not add volume in any meaningful lasting way. They provoke your body into producing its own collagen, over months. You will not see the real result for three to six months. And there is no enzyme that dissolves them. Once the collagen is yours, it is yours.
That trade, patience and permanence in exchange for a result that is structurally your own tissue, is either exactly what you want or exactly what you do not. We would rather you understand it before we inject you than afterward.
Candidacy
Are You a Candidate?
The most important qualification here is not anatomical. It is temperamental.
Biostimulators reward patience and punish impatience. If you need to look different for an event in six weeks, this is the wrong product and we will tell you so rather than take your money and disappoint you. If you are comfortable with a result that emerges quietly over a season, this is one of the most natural-looking things we do.
You are likely a strong candidate if:
- You have volume loss or hollowing, particularly in the temples, midface, or cheeks
- You want jawline definition or hand rejuvenation, which is usually Radiesse territory
- You want a result that looks like your own tissue, because structurally it will be
- You can wait three to six months and you are not chasing a deadline
- You are comfortable with a treatment that cannot be reversed. This one is not negotiable and it is the question to sit with
- You are willing to do the post-treatment massage exactly as instructed, for the reasons explained below
Scope
Where We Treat
We treat
The temples, where hollowing is one of the earliest and least-noticed signs of volume loss. The midface and cheeks, for broad volume restoration. The jawline, for definition and contour, typically with Radiesse. And the hands, where volume loss reveals tendons and veins, and where Radiesse is a well-established option. [CONFIRM: body applications the practice actually offers, and where.]
We do not treat
Sagging tissue and true structural descent, which are surgical. Deep static lines that need filling directly. Skin quality, meaning tone, texture, and pigment, which is a laser or microneedling conversation. And facial movement, which is a neuromodulator. Biostimulators build collagen and do not affect muscle at all.
The Procedure
How It Works, and Why the Massage Matters
Your provider injects the biostimulator into the target area. The session takes 30 to 60 minutes, and the amount of product depends on your anatomy and how much correction you are after.
What happens next is the part that surprises people. With Sculptra, you may look fuller immediately, and then that goes away. That initial fullness is the water the product is suspended in, and it resolves within days. It is not the result. The result is the collagen your body builds over the following months, and it arrives so gradually that most patients notice it in photographs before they notice it in the mirror.
Now the massage. Your provider will tell you to massage the treated area daily. Here is why. Biostimulator particles can clump, and clumping can produce nodules, meaning small firm lumps beneath the skin. Massaging distributes the product evenly while your body is responding to it. It is the single most effective thing you can do to reduce that risk, and it is also the one part of the protocol that is entirely in your hands. Do it exactly as instructed.
Choosing
Sculptra vs. Radiesse: Which Do You Need?
Sculptra
Gradual
- Poly-L-lactic acid (PLLA), a gel suspension.
- Essentially no immediate effect. The initial swelling is water, and it resolves.
- Broad volume loss. Temples, midface, hollowing, and larger areas.
- [CONFIRM: commonly cited as lasting up to 2 years]
Radiesse
Immediate, then gradual
- Calcium hydroxylapatite (CaHA), a thicker gel.
- Provides immediate lift and structure, which then transitions to collagen.
- Contour and definition. Jawline, and the hands.
- [CONFIRM: commonly cited as lasting 1 to 2 years]
The shortest version: if you want gradual, broad volume restoration and you can be patient, that is usually Sculptra. If you want jawline definition or hand rejuvenation with something visible from day one, that is usually Radiesse. Many patients are candidates for both, in different areas, and we will tell you which does what for your face.
Longevity
How Long Do Results Last?
Results are commonly cited as lasting [CONFIRM: one to two years], depending on your metabolism. Because the collagen is your own tissue, it ages as you do.
Recovery
Recovery and Results Timeline
The waiting is the treatment, so here is what the months after actually look like.
Day of treatment
Mild redness, swelling, or tenderness. Most patients return to normal activity immediately. Begin the massage protocol as instructed.
Days 1 to 5
Any initial fullness from Sculptra subsides. This is expected and is not your result disappearing. Avoid retinol, acids, and exfoliants. No intense exercise for 24 to 72 hours.
Weeks 2 to 6
Very little visible change. This is the part patients find hardest, and it is normal.
3 to 6 months
Collagen production increases and the result emerges. Volume, firmness, and improved skin structure.
1 to 2 years
Results are commonly cited as lasting in this range, depending on your metabolism. [CONFIRM]
The hardest stretch is the middle, when the initial swelling has gone and the collagen has not yet arrived, and it feels like nothing is happening. Something is. The result builds so quietly that you will likely miss the day it becomes visible, which is exactly why patients tend to catch it first in a photo rather than the mirror
Your Experience
Your Personalized Treatment Experience
Consultation
We assess your facial structure, where volume has actually been lost, and what you are trying to achieve. We tell you honestly whether a biostimulator is right, whether a filler would serve you better, and whether your timeline is compatible with a treatment that takes months to show.
Treatment
Thirty to sixty minutes. Product selected and dosed for your anatomy, injected by a provider who does this constantly.
The waiting
This is the part of the protocol nobody talks about. You massage daily, you avoid the things on the list, and you wait. Very little happens visibly for weeks.
Results
Collagen builds over three to six months. The result arrives gradually enough that most people around you will not know you had anything done, which is the point.
The Practice
Why Patients Choose Artisan
Founded by a board-certified plastic surgeon
Dr. Diane Z. Alexander, MD, FACS, with aesthetics led by director Lauren Bays. On an irreversible injectable, who is holding the syringe is the entire risk profile.
We offer the alternatives too
Fillers, microneedling, RF, and surgery. Which means when we tell you a biostimulator is right for you, it is because it is, and not because it is the only thing on the shelf.
We will tell you to wait
If you have an event in six weeks, this is the wrong product and we will say so, and we will offer you the one that works on your timeline instead.

FAQ
FAQs
What is the difference between a biostimulator and a filler?
A filler adds volume directly and you see it the same day, and it can be dissolved if you do not like it. A biostimulator adds almost nothing immediately. It provokes your body to build its own collagen over three to six months, and it cannot be dissolved. Different products, different timelines, different commitments.
Can Sculptra or Radiesse be reversed?
No. There is no enzyme that dissolves biostimulators the way hyaluronidase dissolves hyaluronic acid fillers. This is the most important thing to understand before you consent, and it is why conservative dosing and an experienced injector matter so much.
Sculptra or Radiesse?
Sculptra is purely gradual and suits broad volume restoration, temples, midface, and hollowing. Radiesse provides immediate structure that then transitions to collagen, and suits jawline definition and hands. Many patients are candidates for both, in different areas.
When will I see results?
Three to six months. Any fullness you see in the first days after Sculptra is the water the product is suspended in, and it resolves. That is not your result. The collagen is.
What are nodules, and how likely are they?
Small firm lumps that can form if the product clumps. They are the best-known complication of biostimulators, particularly Sculptra. Most resolve, some require treatment, and the massage protocol exists specifically to reduce the risk.
How much does it cost?
Biostimulators are priced by the vial, and most plans use several vials across multiple sessions. We give you the total across the series at consultation, so you can plan for it. Financing is available.
Why do I have to massage the area?
Because biostimulator particles can clump, and clumping can create nodules, meaning firm lumps beneath the skin. Massaging distributes the product while your body responds to it. It is the most effective thing you personally can do to reduce that risk. Do it exactly as instructed.
How many sessions will I need?
Commonly up to three, depending on how much volume you are restoring. Your provider will build the plan at consultation.
How long do results last?
[CONFIRM: commonly cited as one to two years, depending on metabolism.] Because the collagen is your own tissue, it ages as you do.
Will this stop my wrinkles from moving?
No, and if you want that, you want a neuromodulator. Botox, Dysport, and Xeomin relax muscle and reduce movement. Biostimulators build collagen and do not affect muscle at all. They are entirely different treatments and are frequently used together.
Is this the same as microneedling?
No, though the naming causes real confusion. Microneedling injures the skin superficially to trigger collagen and improve texture. Biostimulators are injected and rebuild volume and structure. Both build collagen, by completely different means, and they are often combined.
I have an event in six weeks. Can I do this?
You can, and you will not see a result by then. If your timeline is that short, a filler is the honest answer, and we will tell you so rather than sell you something that cannot do what you need.
The best version of this result is the one nobody notices arriving.
Come in and we will tell you whether a biostimulator is right for your face, your timeline, and your tolerance for a decision that cannot be undone, including when the answer is something else entirely.
